Purpose:

The provision of a Collaborative Reablement Service. Reablement forms one of the care services, alongside short-term crisis response services, intermediate care services delivered by health professionals and bed-based intermediate care services.  It is a short term (up to six weeks) intervention following hospital discharge or after an emergency at their own home in the community, the aim of which is to help people to gain skills to continue living in the community as independently as possible by learning or relearning skills necessary for daily living.  

Reablement services work closely with other services and disciplines as part of a multi-disciplinary team and is a non-chargeable service for anyone who is able to benefit from it. 

Decision:

Following the conclusion of the tender process, it was agreed that the contracts will be awarded and commence on 1st October 2021.

 

Reasons for the decision:

Adult Social Care has specific duties under the Care Act 2014 to provide services to vulnerable adults. This service enables and supports people to remain independent and live in their own homes for longer.
